HOW IS THE BASIC SUM ASSURED OF MY POLICY OBTAINED? Your policy's Basic Sum Assured is obtained according to your entry age (last birthday age), gender and the number of units of insurance coverage that you want to purchase (Refer to above Premium Table and Sum Assured Schedule). For example, a Female age 50 years old (last birthday) who has purchased 5 units of insurance coverage under Senior Gold plan, her Basic Sum Assured will be 5 times of the amount stipulated in the Sum Assured Schedule above; i.e. RM17,397 x 5 = RM86,985. |

- Preferred Premium Amount and Mode of Payment (Monthly, Quarterly, Semi-annually or Yearly)

- How would you like to make your initial premium payment? (By Credit Card or Cheque or Direct Bank-in to Alliance Bank) Payment by CASH is not recommended.
