6.2 Change of Applicant / Owner

In the event of the earlier death of the applicant / owner before the insured

Request for Change Form (x 2). Adult Policies: Signed by the administrator / executor of the applicant / owner's estate as stated in the Letter of Administration (LA) / Probate. Juvenile Policies: Signed by the Contingent Beneficiary
Death Certificate of the applicant / owner

In case of changing the assignee, the requirements are:

Release of Assignment Form
Assignment Form (Absolute or Conditional)
Stamping Fee of RM20

For juvenile policies, upon the death of applicant / owner, the contingent beneficiary will automatically be the new applicant / owner before the insured attains the age of majority.
