5.1 Change of Policy Plan
  1. Request for Change Form (x 2)
  2. Health Declaration Form
  3. Payment of premium
  4. Policy contract
  1. Participating / Non-Par plan to Participating plan: within 1st year only
  2. Participating / Non-Par plan to Non-Par plan: allowed at any premium payment date
  3. Change of Permanent Plan to Term Plan is not allowed
