5. Dread Disease

5. Dread Disease / Critical Illness Claim	Written notice of Claim must be given within 6 months from the initial diagnosis of the disease.	Dread Disease Claim Form which include:- i) Claimant's Statement, ii) Confidential Medical Certificate (based on type of disease) and iii) Histology report for Cancer cases. Policy Contract	Upon payment of any dread disease claims, other supplementary contracts will be terminated. Any statement, medical reports, etc. are to be furnished at the Claimant's expenses.
