3. Hospitalisation & Surgical Benefits (HS & HB)

Written notice of Claim should be given within 10 days from the date of commencement of confinement in the hospital.

Accident and Hospitalisation Claim Form
All original bills / receipts (Photostated copies of bills / receipts certified by MAA's officer are also acceptable but only on certain cases)

Part II of Individual Hospitalisation Claim Form may be waived if the claim is for RM200 or less, provided the hospital Discharge Certificate presented indicates:

Date of admission
Date of discharge
Diagnosis

Any fees incurred in obtaining the Certificate of Medical Attendant and the Discharge Certificate are borne by the insured.
