Written notice of Claim Form should be given to the company within 20 days after the date of accident.

Accident and Hospitalisation Claim Form
Police Report
Medical Certificate
Letter from Employer (if applicable)

Part II of Individual Hospitalisation Claim Form may be waived if the claim is for RM200 or less, provided the hospital Discharge Certificate presented indicates:

Date of admission
Date of discharge
Diagnosis

Any fees incurred in obtaining the Certificate of Medical Attendant and the Discharge Certificate are borne by the insured.
