LIFE POLICY CLAIMS PROCEDURE


1. DEATH

Claim Type Notification Documents/Information Remarks
1.Death Immediately notify and get necessary forms from the nearest branch personnel executive and where possible quote:
  1. Policy Number
  2. Proof of Death (e.g. 'Permit Menguburkan' or Death Certificate)
  3. Deceased's full Name and Address
  4. Date / Cause of death
  5. Name and address of Claimant
  1. Claimant's Statement
  2. Physician's Statement
  3. Death Certificate
  4. Policy Contract
  5. Consent on Release of Medical Record (when applicable)
  6. Proof of Age (if not admitted)
  7. Proof of Relationship between the claimant and the deceased (e.g. Birth Certificate, Marriage Certificate)
  8. Letter of Administration / Probate (when applicable)
  9. Statutory Declaration to confirm the deceased is not married (if beneficiary under estate)

When death was resulted solely from an accident, the additional reports required are:

  1. Police Report
  2. Coroner's Report and/or
  3. Post Mortem Report
  4. Newspaper cutting (if available)
  1. Natural Death
  2. Accidental Death
  3. Suicide - Principle Sum Insured is payable if the policy has been in force for more than 1 year.

Proper Claimants:

  1. Nominee(s). If no nomination please refer to Appendix.
  2. Trustee(s)
  3. Policy Owner (Third Party)
  4. Assignee
