IMPORTANT NOTES

Please return the completed form together with the required premium as soon as possible. Part of the form will be returned to you as prove of receipt of premium from your good self. If your child is currently covered by existing Master Policy held by the School which is expiring soon, it is necessary to complete this form and return it together with the required premium to the School so that coverage can be arranged for your child for a further year.

In the event of discrepancy, ambiguity and conflict in interpreting any terms or conditions of the contract, the English version shall prevail and supercede the Bahasa Malaysia version.
