CONDITIONS

1. Claims for Benefit 3 (Medical Expenses) must be supported by original medical bills from registered medical practitioners and/or hospitals.

2. Accidental Death or Permanent Disablement must occur within twelve (12) calendar months from the date of accident.

3. Compensation for Benefit 2 (Accidental Permanent Disablement) shall be payable only if such permanent disablement has lasted for a period of twelve (12) months from the date of accident and at the expiry of that period, such disablement is beyond hope of recovery and will continue for the rest of the insured person's life.

4. This policy shall cover eligible persons aged 4 to 18 years old (age next birthday) only.
