Divers, Police, Army/military and law enforcement officers, Aircraft testers, Pilots or crew, Seamen and sea fishermen, Racing drivers, Jockeys, Oil rig workers, Sawyers and timber logging workers, Firemen, War correspondents, Steeplejacks, Stevedores, Persons engaged in demolition of buildings, Persons engaged in ambulance services, Woodworking machinists, Explosive handlers, Underground tunneling and mining, and Professional sports activities.

If you wish to apply for the FleXi PERSONAL ACCIDENT coverage, just follow the following instructions:-

1. OBTAIN A PROPOSAL FORM
  • You can print a proposal form by clicking here ; or
  • Request a proposal form via email and we will email you the proposal form; or
  • Call/SMS 012-6597262 for a proposal form.
2. SUBMIT THE PROPOSAL FORM AND MAKING PAYMENT
You may choose any of the following ways to submit your completed form:-
  1. By FAX - Koo Agency Fax No.: 03-80249046; or
  2. By POST - Mail to Koo Agency, AS-00-04, SP Goodyear Court 10, 47630 Subang Jaya

You may also choose your preferred way of payment as follows:-

  1. By CREDIT CARD (Visa or MasterCard) - Print a copy of this "Credit Card Instruction Form" and enclosed the completed form together with the proposal form; or
  2. By CHEQUE - Enclosed your payment cheque together with the proposal form. Kindly make cheque payable to "Malaysian Assurance Alliance Berhad"; or
  3. By DIRECT BANK-IN or ONLINE TRANSFER - Direct bank-in or online transfer to either i) Koo Agency MAYBANK Account No.: 5123-611-44-193 or ii) Koo Agency ALLIANCE BANK Account No.: 1211-800-1000-4155. If by bank-in, please fax a copy of the bank-in slip together with the proposal form.
