Class
1 : Persons engaged in professional administrative, managerial, clerical and
non-manual occupations.
Class
2 : Persons engaged in work of supervisory nature but not involved in manual
labour.
Class
3 : Persons engaged either occasionally or generally in manual work which
involves the use of tools or machinery.

SUBMIT THE PROPOSAL FORM AND MAKING PAYMENT |
You may choose any
of the following ways to submit your completed form:-
- By FAX

- By POST
- Mail to Koo Agency, AS-00-04, SP Goodyear Court 10, 47630 Subang
Jaya
You may also choose your preferred way of
payment as follows:-
- By CREDIT CARD
(Visa or MasterCard) - Print a copy of this "Credit
Card Instruction Form" and enclosed the completed form
together with the proposal form; or
- By CHEQUE
- Enclosed your payment cheque together with the proposal form. Kindly
make cheque payable to "Malaysian Assurance Alliance Berhad";
or
- By DIRECT BANK-IN or ONLINE
TRANSFER - Direct bank-in or online
transfer to either i) Koo Agency MAYBANK Account No.: 5123-611-44-193
or ii) Koo Agency ALLIANCE BANK Account No.: 1211-800-1000-4155. If by
bank-in, please fax a copy of the bank-in slip together with the
