CLAIMS PROCEDURES

  • Report the incident immediately to MAA (Call Cantre 1-300-88-8622) to obtain the FWHS claims form.
  • Complete and submit all relevant documents as required in the Claim Form as soon as possible in order not to delay processing the approval of the claim settlements.
  • Upon completion of submission of all relevant documents, the reimbursement of the claims shall be made within 30 days by MAA.
  • Any enquiries pertaining to claims matter shall be addressed to MAA.
