Report the incident immediately to MAA (Call
Cantre 1-300-88-8622) to obtain the FWHS claims form.
Complete and submit all relevant documents as
required in the Claim Form as soon as possible in order not to delay
processing the approval of the claim settlements.
Upon completion of submission of all relevant
documents, the reimbursement of the claims shall be made within 30 days by
MAA.
Any enquiries pertaining to claims matter
shall be addressed to MAA.
