APPLICATION PROCEDURES
- You may request for the SKHPPA Proposal Forms by
instructing us via email or call/SMS
012-6597262 providing us your Company's Name, preferred postal
address and contact numbers.
- The Employer must complete the proposal form
and submit or post to us the completed proposal form together with the
premiums (For payment by CHEQUE, kindly made cheque payable to
"Malaysian Assurance Alliance Berhad") to Koo Agency, AS-00-04, SP
Goodyear Court 10, 47630 Subang Jaya.
- Coverage is effective upon receipt of payment
of premium and the proposal forms. Kindly inform us of your posting.
- The Policy will be sent to the employer within
30 working days.
- If the policy has not been received within the
stipulated period, kindly instruct us.
