To enrol under the FWCS, just complete the FWCS Proposal Form which can be obtained by instructing Koo Agency by email or call 012-6597262 and return the completed form together with:

1. Copy of Passport of each foreign worker

2. Copy of Work Permit of each foreign worker

3. Premium Cheque made payable to "Malaysian Assurance Alliance Berhad" as the approved insurance company by the Ministry of Human Resources.
