Claim Procedure

In the event of an accident covered under the FWCS:-

1. REPORT the accident immediately to the Labour Department (applicable to employment accident only).

Failure to report the employment accident shall render the employer guilty of an offence under Section 13(5) of the Act and shall be liable to a fine not exceeding RM5,000 for a first offence, and a fine not exceeding RM10,000 for any subsequent offence.

2. REPORT the accident immediately to MAA Assurance and submit the following documents (whichever is applicable) for claims processing:-

  1. Completed Claim Form

  2. Labour Assessment

  3. Medical Report

  4. Medical Receipts

  5. Death Certificate (for death claim)

  6. Post-Mortem Report (for death claim)

  7. Police Report (for death claim)

  8. Other Claims Documents (if any) as required
